ASSESSMENT-BASED TREATMENT REFERRAL & PLACEMENT

This service connects individuals to appropriate care based on your history, experience, strengths and needs, preferences, and goals via a thorough assessment. All referrals made are separate from myself or my business and I do not receive financial gain. This service can connect you with referrals such as:

• Substance Abuse Inpatient and Outpatient Treatment

• Mental Health/Dual Diagnosis Inpatient and Outpatient Treatment

• Family Education

• Specialized Treatment e.g. Eating Disorders, Faith-Based, LGBTQ+, Gender Specific, Psychiatric, etc.

• Community Outreach Centers

• Sober Living Environments

• Licensed therapist with specialty based on your needs

• Homeless shelter, or transitional housing for low-income unhoused individuals

• County/State funded programs serving low-income families

• Education or Occupational Development Resources

• Referral to Intervention Specialist (if needed)

• Referral to Sober Coach/Companion (if needed)

All the above services are entirely free, with the exception of those listed here:

• Individual sessions aside from initial consultation or referral resource appointment

• Intervention Specialist once services are requested from them (separate entity)

• Sober Coach/Companion once services are requested from them (separate entity)
